Show clear yes or no for checkboxes instead of NA where an answer exists

Users have reported that the HTML version of the 4-point report shows "NA" (Not Applicable) for checkboxes that are answered (e.g. "yes") in the PDF version, which is confusing. Many critical checkboxes on the 4-point report are incorrectly reading "NA" when they should display "yes" or "no". Users want a cleaner way to display checkbox answers as clear "yes" or "no" so the HTML and PDF match and readers are not misled.
